I really like your examples of the various forms. Would it be possible or does a site exists that will show the process of making your own forms. I am not a program and after trying the directions to covert a Word file don't think I understand the XML part.

We are making video demo showing main steps in creating your own form.
The video should be available in a few weeks.

But, if you need professionally looking form, and don't have enough experience, I'd recommend to contact our Sales team and request an estimate of how much it will cost for us to build the form for you.
